Basically a bleed under the fingernail which is under high pressure as there is no way to escape. Normally after jamming your finger in a car door. The pressure is released via a small hole drilled using a needle. Another way is to heat up the tip of a paperclip and it will melt through the nail, releasing the pent up blood.
